Through TennCare-administered programs and federal waivers, our services are designed to build independence, foster meaningful relationships, and support each person’s unique journey.

Employment and Community First CHOICES (ECF)

Helping individuals find purpose through work, daily life skills, and social inclusion.

  • Community Integration Support Services (CISS)
    Participation in volunteering, recreation, and cultural activities that build friendships and belonging.
  • Independent Living Skills Training (ILS)
    Hands-on training in personal hygiene, cooking, budgeting, community safety, and technology use.
  • Personal Assistance (PA)
    Help with bathing, eating, mobility, hygiene, and routines at home and in the community.
  • Supportive Home Care (SHC)
    In-home care and support with daily tasks like dressing, housekeeping, and meal prep.
  • Community Living Supports (Levels 1a–4)
    Tiered services to promote independent living, tailored to each individual’s level of need.
  • Employment & Community Support Navigation
    Helping individuals explore job opportunities and navigate vocational supports.
  • Additional Services:
    Peer-to-peer self-direction, decision-making supports, transportation, and more.

    Health monitoring, medication support, case management, and behavioral services.
  • Personal Assistance & Emergency Response Systems (PERS)
    Daily support and safety monitoring.
  • Transportation Services & Non-Residential Supports
    Helping individuals stay connected and independent in their communities.

Katie Beckett Waiver – Parts A & B

Serving children with disabilities who don’t qualify for traditional Medicaid.

  • Part A:
    Medical support for children needing high-level care at home.
  • Part B:
    Community-based services including supportive home care, integration activities, and transportation.

CHOICES Program

Helping adults with disabilities or seniors stay safely in their homes or communities.

  • Community Living Supports (CLS) – Helping individuals thrive at home.
  • Personal Care Services – Assistance with hygiene, meals, and mobility.
  • Transportation Assistance – Ensuring access to essential services.
